I'm still too timid to try darker values after past failures, but at least this worked out better.  Maybe next time ;)

Google Forms

I'm not sure how long the 'new & improved v. of google forms has been out, but apparently that is yet another victim of the big brand devolution trend; having dropped it's most useful functionality (to me) and is no longer capable of auto-populating a form starting with an existing google spreadsheet.
